Student Health Requirements

Student Health Requirements

School Physicals

Physical exams are required for students entering school in grade 6, grade 11, or entering a Pennsylvania school for the first time.

Dental Examinations

Dental examinations are required for students entering school, grade 3, and grade 7. Children transferring from other school systems must submit a dental exam form if the original school does not provide an adequate health record.

If your child has received a dental examination, please have your dentist complete the Dental Form and submit it to your child's school office.

Medical Forms

For medical, asthma, and other health forms that may apply to your child, please visit the Nursing Services webpage.

Immunizations

According to Pennsylvania state law, students must provide documentation of required immunizations within the first five (5) days of the 2026-27 school year. According to the School District calendar, Day 5 will be Wednesday, August 26, 2026.

Students who have not provided the required documentation to the school nurse by Wednesday, August 26, at 4:00 PM, may be excluded from school as of Thursday, August 27, 2026, until appropriate documentation is provided to the school.

To avoid disrupting your child’s education, we are providing you with a list of required immunizations to share with your medical provider to obtain documentation of all required immunizations or to schedule an appointment before the August 26, 2026, deadline.

If you have specific questions about your child’s immunization status, please contact your child’s medical provider. We appreciate your assistance in obtaining the required immunizations as we comply with state mandates.
