Sports Physicals & RankOne System

Sports Physicals

Each school year, all student-athletes are required to obtain a sports physical from their primary care physician or an urgent care facility, dated after May 1, 2026. They are also required to re-certify their physical information each sports season if the physical is dated more than six weeks before the start of the season.

This is a mandatory requirement that MUST be completed at least 2 days before the start of official practice to be eligible for day 1. This includes any sport that is conducting official tryouts.

Required Physical Form to be Completed by a Physician & Submitted to the Athletic Training Office:

  • Print a hard copy of the PIAA Sections 5 and 6 Health History and the Medical Examiner pages to take to the doctor's office to be completed. Only this physical paperwork from PIAA is acceptable. All other forms will be rejected.

  • MUST be submitted to the Athletic Training Office.

  • Both papers must be returned together.

RankOne

Any student planning to play a fall sport MUST submit their information via the Rank One system.

Reminder: RankOne information cannot be updated before July 10, 2026, due to the system rollover for the new school year.

If you have questions after reading the directions linked below, please contact one of our Athletic Trainers, Marina Rohrbaugh at RohrbauM@sgasd.org, or Paige Landes at LandesP@sgasd.org.
