Required Back-to-School Forms
Back-to-School paperwork is available in Sapphire for parents/guardians to complete. When you log in to your Sapphire account, the required forms will appear on each child's home page.

Required Forms Awaiting Completion – Please complete by September 11th:
- Handbook Acknowledgment
- COPPA/FERPA Acknowledgment
- Permission to Administer Tylenol/Antacid (Grades 5 – 12 only)
- iPad Protection Plan Choice
- Student Data Card
Optional Forms
Optional forms are NOT required to be submitted and are available in Sapphire's Student Data Forms section if you elect to submit them.
